Our Policies Regarding Dental Insurance Many of you may have orthodontic coverage as part of you benefits package. Please let our treatment coordinator know all of your insurance
information before or at your first visit. This information includes the name, address, employer, birth date, social security number, and group number of
the insured and the name, address, and phone number of your insurance company. In most cases we will accept assignment of benefits from your primary and secondary insurance carriers. Should you change insurance
coverage during treatment please let us know immediately; before the change takes place if possible. Some plans will not cover treatment already
in progress. Please bring any forms requiring Dr. Heller's signature to us as soon as possible to assure that your plan pays the benefits you are entitled
to. When your financial arrangements are worked out with the treatment coordinator, your estimated benefits will be subtracted from your total fee.
Should your insurance not pay the entire estimated amount, you will be required to pay the difference prior to completion of treatment. Plan Participation
Participation in preferred provider and HMO type dental plans requires following the insurance plan's fee schedule. In many cases this fee is a
significant reduction from our regular fee and would necessitate skimping on the quality of treatment. For this reason, Dr. Heller is selective about
participation in these types of plans. Below is a list of plans the office currently contracts with. If your plan is not on the list, please inquire with the treatment coordinator.Aetna US Healthcare PPO
